The University of Miami Health System, "UHealth", Department of Pediatrics- Mailman Center for Child Development has an exciting opportunity for a Full-Time Patient Navigator to work in Miami.

The Patient Navigator facilitates patients’ ability to obtain services by identifying barriers to care, coordinating the development of treatment plans, and monitoring the implementation and follow-up of services ordered by caregivers.

Core Responsibilities:

  • Conducts patient evaluations to obtain information necessary for the development of an effective treatment plan.

  • Ensures patients have access to insurance and social service programs for which they are eligible. Provides patient reminders of upcoming appointments and acts as liaison between patients and providers.

  • Identifies and resolves real or perceived barriers to care.

  • Educates and counsels patients on issues related to their health and life situations.

  • Maintains current knowledge of community health, welfare resources, and any other programs available to patients.

  • Handle patient communications and contact healthcare facilities for medical records.

  • Address care gaps with the nurse navigator.

  • Prepares, tracks, and processes medical records and pathology specimens. Reviews and file medical records for completeness; use tracking systems for accurate documentation.

  • Provides administrative support to the department by answering phones, address inquiries, and direct calls or take messages, Copy, fax, emailing, and mailing records while ensuring confidentiality.

  • Adheres to University and unit-level policies and procedures and safeguards University assets.

Department Specific Functions:

The Patient Navigator will be responsible for:

  • Conducting intakes (6-8 per week) to a caseload of about 80 active families. (Throughout the year about 250 total).

  • Complete a SWOT and Action plan to provide resources to low need families (families with short term needs)

  • Complete contact logs (documenting calls with families) weekly

  • Ensure family is receiving service requested (acts as liaison between provider and family)

  • Discuss cases during group meetings/staffings

  • Keep and review records and files

  • Has knowledge of current available services in the community

  • Attends all trainings/meetings

  • Some driving may be required, as intakes could be conducted in the homes, childcare centers, places of work and location most convenient for the family.  The employee must have a valid drivers license and a vehicle to be successful in this position.

CORE QUALIFICATIONS:                                                                                                    

  • High School diploma or equivalent. Associates or Bachelors in related field preferred.

  • Minimum 1 year of relevant work experience

  • Valid Driver's License

  • Bilingual in either Spanish or Creole preferred

  • Ability to process and handle confidential information with discretion.

  • Ability to communicate effectively in both oral and written form.

  • Ability to handle difficult and stressful situations with professional composure.

  • Ability to maintain effective interpersonal relationships.

  • Ability to understand and follow instructions.

Any appropriate combination of relevant education, experience and/or certifications may be considered.
